I, the undersigned (captain name), as team captain and on behalf of its registered players, pledge that our health allows us to take part in the sport we have registered for, and that we bear full responsibility for any medical injury resulting from poor physical fitness.

Declarations

Both players are citizens, children of citizen mothers, or born in the UAE. The team is two players within C Open, and rankings are matched against the regulation before approval. We will play under the approved doubles rules.
